Windent HIPAA Agreement
Our standard HIPAA agreement is now available on the web. You can download our agreement, fill out your information, print, sign and fax back to us.

1. Click the Windent HIPAA Agreement link above.
2. Click in the fields for Doctor's name, address, date, signed by, etc. and type your information.
3. Click File -> Print.
4. Sign the agreement where indicated.
5. Fax to Windent, 214-346-0919.
